The NDIS exercise program we have developed is aimed at doing just that!

Unlocking your Independence
What we have found with each of our participants is that we help to unlock their most independent self through focusing on developing their Confidence, Connection and Participation
Building Confidence includes developing and/or refining skills and supporting each participants with their level of need.
Building Connection includes building a safe social space and supporting each participants with their level of need.
And building Participation includes developing and/or refining skills and building a safe social space.
Skills consist of many different aspects from the strength, stability and balance to, ability to do tasks of daily living and communication skills.
Supporting each participant with their level of need breaks down into personalising goals, learning styles and what is enjoyed, as well as self management of appointments, home exercise or even what happens within the appointments them selves.
The Social aspect of the program is building relationships with a mentor or coach (in the Exercise Physiologist taking the participant), fostering team work and social expectations and finally being part of the Tailored Health community from attending inclusive events and a welcoming clinic atmosphere.
What can I expect from the first five appointments?
Appointment 1 – Get to know you/us
This appointment is all about getting to know you (the participant) and you getting to know Tailored Health and your practitioner, your goals (personal and NDIS) and a primary assessment of your ability/skills.
Appointment 2 – Find your moves
This appointment is to continue to getting to know you, finish any assessments required and create a Home Exercise Program (HEP) (where applicable)
Appointment 3 – Show your moves
This appointment is all about you showing what you have learnt so far and continuing to develop your goal specific skills
Appointment 4 – Grow your moves
This appointment is all about improving upon the goal specific skills you have learnt so far
Appointment 5 – Measure your moves
This appointment is all about reassessing from your initial appointment to make sure we are progressing towards your goals.
